Application Process for CHW Tumour Bank

Stage 1:- Contact the Tumour Bank via email (tumourb@chw.edu.au), surface mail (Tumour Bank - Clinical Sciences, The Children's Hospital at Westmead, Locked Bag 4001, Westmead 2145, Australia), or fax (02 9845 3078) detailing the type of specimens you require.

Stage 2:-You will receive a reply from the Tumour Bank detailing the number of cases, and the amounts of each specimen stored, which match your requirements.

Stage 3:- Read the Conditions of Use of Tumour Bank Samples and complete the CHW Tumour Bank Application Form ensuring you include proof of Ethics Committee approval.

Stage 4:- Your application will be reviewed by the Tumour Bank Committee which consists of two Research Scientists, a Pathologist and an Oncologist. In particular, the application will be assessed for its perceived value in relation to the demands placed upon the Tumour Bank. Additionally, as the specimens have taken several years to collect, the Tumour Bank Committee will be concerned about maximum utilisation of the specimens. You will be notified in writing whether your application has been successful.

Stage 5:- Once your application has been approved and you have demonstrated a source of project funding, you will be requested to sign an agreement for the use of Tumour Bank samples (see Agreement for CHW Tumour Bank Sample Use & Acknowledgment and Agreement for CHW Tumour Bank Sample Use & Collaboration). Once the signed agreement has been received, an arrangement will be made to transport the specimens to you at your own expense. There are no other charges for the provision of samples from the Tumour Bank.

If you are applying for funding from a large, peer-reviewed granting body you are advised to contact the Tumour Bank well in advance of the final date for grant applications. Applications for Tumour Bank samples will be considered if funding is pending.

Applications for Tumour Bank specimens will be reviewed in February, April, June, August, and October.

Applications for use of Tumour Bank specimens will not be considered unless a completed application form has been received with the required documentation.
