• Provides discharge planning for children with orthopaedic conditions.
• Ensures that the child and family are educated on the necessary requirements for home care and identifies potential problems in order to facilitate early discharge.
• Co-ordinates community support services for families with children that have mobility restrictions.
• Can provide home and school visits to assess children?s needs within the home and school.
