| Location: | Children's Assessment Centre, Outpatients Building, Level 3 | The Clinical Nurse Consultants work within the multidisciplinary Rehabilitation Team, and specialise in the area of brain injury in children. The CNC for Inpatient Rehabilitation is involved with the child and their family during their inpatient stay, and addresses the discharge planning issues. The two other CNCs on the team are involved with the child and family on discharge, and continue to provide support and coordination of outpatient services for the family and facilitate their transition from the Hospital setting back into the community. Outpatient referrals:Contact Rehabilitation Secretary on 9845 2132 who will put you in contact with the current Intake Officer.
