Medical RecordsThe Medical Record Department (MRD) maintains the integrity and security of patient records and the information held within them. Patient information may be accessed for ongoing patient care, research, legal purposes and for patient access in keeping with the Health Records and Information Privacy Act (HRIP) 2002. The Department is also responsible for coding all patient separations using the ICD10-AM classification system for the NSW Inpatient Statistics Collection, processing applications for access to patient information and typing outpatient clinic letters. Government Information (Public Access) Act 2009 (GIPA) requests are coordinated through the MRD's Right to Information Officer. Those wishing to apply for copies of or access to their personal health information under Department of Health policy are required to complete the appropriate form and mail to the address detailed on the form. Instructions, including the appropriate fees and identification requirements are detailed on the form. Patient records are then generally released under the Health Records and Information Privacy Act 2002.
