Case Manager
The Case Manager forms part of the Brain Injury Service team that may become involved with the child/young person's care once they are discharged.
They form the link between the family and the community and can address the following:
- Co-ordinating ongoing appointments.
- Assisting with the child/young person's return to school.
- Accessing ongoing therapy.
- Accessing home support, respite and community services or leisure activities.
- Liasing with insurance companies. If the child/young person has an insurance claim, the Case Manager is required to submit a plan to the insurance company describing the child/young person's progress and goals.
Contact may be made by phone calls, home visits and attendance at hospital appointments. The length of time a child/young person is case managed depends on the child/young person's and family's needs. For more information, please go to Case Management.
