"Graduation Ceremony" for Patients Transitioning to Adult Service

Transition planning and moving onto adult services is an important step for young people becoming independent and managing their own health care. Therefore, children and their families who have had a long association with The Children's Hospital at Westmead are invited to be a part of the 2010, Graduation Ceremony. This is targeted at patients who have or will transition to adult health services this year.

2011 Graduation ceremony details are yet to be determined.
