Do you care for a young person 15yrs or older?

Have you thought about what health care services they will need when they leave the Children's Hospital? Did you know that we have a Transition Coordinator at CHW?

Did you know there are adult transition coordinators who can help in planning the move?

To help you prepare and plan for future health care needs talk to your health care team, or come along to an information evening.

Who's Invited?

• Parents/Carers of young people 15yrs or over, with a chronic health condition who will require ongoing care in adult life.
• Parents/Carers of young people 15yrs or over, with a health condition that will not allow them to manage/live independently in adult life.
• Young people who would like to ensure they feel well prepared about the adult system and who would like to meet others who have already taken that step.
• All Staff of the Children's Hospital.

How do I register to come?

Registrations should be received 1 week prior to the event for catering purposes. Contact Receptionist, Dept of Adolescent Medicine on 9845 2446, or Bin Moore Transition Coordinator.
