How do I make a referral to the Eye Clinic?
The Eye Clinic at the Children's Hospital Westmead, offers eye examinations and treatment of eye disease for children from birth to 16 years of age. To make a referral to the Eye Clinic the referring doctor should:
- Identify the possible eye problem
- Refer the child using the Eye Clinic referral form
. Please ensure you fill out all fields with detailed information on reason for referral.
- Then fax this form to (02) 9845 3949.
- If the referral meets our criteria for an appointment, the child's family will then be sent an appointment letter.
- Please note we are a tertiary referral centre and do not have the capacity to see all children. If a child is referred for a condition that does not meet the Eye Clinic referral criteria, the referring doctor will be notified and other options for care provided.
What should I do if the referral is urgent?
The Ophthalmology registrar should be contacted through the Children's Hospital switchboard on 9845 0000. The child & family should be sent to the Accident and Emergency Department, Children's Hospital, Westmead. Please send a referral letter with the patient.
